Cwm Taf University Health Board are offering an exciting and challenging opportunity to work as a Nurse Assessor within the Cwm Taf foot print.
We are looking for a professional who is reliable, flexible in their approach and self motivated but who can also work well within the Nursing Team.
You will support the Nursing Team in undertaking individual nursing assessments and reviews, co-ordinate and undertake Multi-Agency Disciplinary meetings to establish an individual's eligibility for NHS Continuing Health Care and Funded Nursing Care, by involved in training staff on the implementation of the Continuing Care Framework.
You will be working closely with colleagues from Local Authorities, undertaking Joint Monitoring Visits and providers from the Independent private Care Home Sector.
The ability to speak Welsh is desirable for this post
