An exiting opportunity has arisen to join the Discharge Liaison/ Complex Care Team within the Localities Directorate of Cwm Taf University Health Board.
The post will cover all sites within the CTUHB areas. however there will be a requirement to cover across any of the Hospital sites to cover leave arrangements as and when required, in line with supporting your colleagues caseloads during their periods of absence.
As a Band 6 Complex Care Liaison Nurse you will be expected to have a wide range of knowledge and skills in relation to Delayed Transfer of Care [DToC]; Continuing Health Care [CHC]; Funded Nursing Care [FNC]; Choice of Accommodation and patients who require long-term care; patient assessments in relation to requirements for rehabilitation and packages of care.
You will be required to have sensitive discussions with patients and their families and yet challenging but diplomatic conversations on occasions in order to achieve the best outcome for patients with discharge at the point of medical fitness being the main focus.
Employing excellent communication skills with a range of multi-disciplinary team members will be key to this role.
The ability to speak Welsh is desirable for this role.
