Cancer Nursing Health Care Support Worker/Navigator

Do you have the passion and drive to be the Cwm Taf Cancer nursing health care support worker/navigator? Could you work alongside the Lung cancer team to ensure the patient’s holistic needs are met? Do you enjoy challenge and the opportunity to innovate?

This individual will support the Clinical nurse specialists (CNS) for lung cancer, in guiding patients and their families through their cancer journey, signposting them to the relevant available support. A key element of the role will be ensuring co-ordination of care for lung cancer patients across the cancer pathway from diagnosis through to end of life.

As a member of the specialist cancer team, the post holder will provide support and care to the holistic needs of adult cancer patients, ensuring that cancer patients have support in navigating through the often complex health and social care systems.  This will ultimately improve the cancer patient experience.

As this role is new and innovative, the successful candidate will need to develop the scope of the role and capture outcomes.

This is a 6 month fixed term/secondment.

The successful candidate will be; required to undertake a range of assigned tasks involving direct and indirect care for the benefit of the cancer patient in accordance with agreed protocols; be well organised having good time management skills and the ability to work as part of a team or on their own initiative and have good communication skills.

 

The ability to speak Welsh is desirable for this post.
