Do you have a passion for community nursing and are looking to develop your knowledge and skills within this field. If so, we currently have band 5 vacancies within our Community Hospitals in CTUHB, where free car parking is available.

The team of qualified nurses and health care support workers provide nursing support to adults with rehabilitation needs and support complex discharge planning, ensuring that patients are discharged to the most appropriate destination.

We are looking for compassionate, motivated and reliable individuals who have a desire to care for adults in the community hospital setting. Within this role you will be responsible for caring for patients in addition to providing ongoing support and training for healthcare support workers engaged in the delivery of care.

The teams within the Community Hospital support the acute service, which allows for discharge planning. We work closely with other organisations and are continuously looking to develop innovative, integrated and comprehensive healthcare services that support our patients. The work is varied and requires a good range of clinical skills and offers a degree of autonomy within a supportive and friendly environment. The post holder will be part of an integrated team responsible for the planning, implementation and evaluation of nursing care needs and the delivery of high quality care. You will be supported in your role by the Ward Manager, Deputy Ward Manager and the Senior Nurse.

You will need to be motivated with a wish to excel in patient care. Our services are working towards seamless patient pathways using a patient centred approach.

We believe in your development and support staff with preceptorship, mentorship and clinical supervision. A comprehensive induction and ongoing training will be provided.

We encourage informal visits to allow applicants to familiarise themselves with our services and to answer any questions they may have.
