An opportunity has arisen for three highly motivated band 6, Mental Health Practitioners to join our team within Specialist Dementia Intervention Team (SDIT). The successful candidates will be expected to undertake assessment formulations and develop a plan along with the MDT to support those suffering with behaviour that challenges while living with Dementia. You will work collaboratively within team ensuring deliver of high quality care to those living in the community and care home environment. The team offers a 12 week, intensive intervention programme and you will be expected to be involved in all aspects of this work. In addition to this you will be expected to offer daily support to HCWS staff and others involved within the team. The team will operate on a 7 day rota with morning and afternoon shifts in operation.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
