An rare secondment opportunity has become available, for a highly experienced physiotherapist to join Cwm Taf’s At Home Service based in Dewi Sant Hospital, Pontypridd. We are looking for an enthusiastic physiotherapist who wants to explore their potential to work in this setting. We are looking for a physiotherapist with exceptional interpersonal skills to work within an expanding and very supportive multidisciplinary team.
The post holder will work in domiciliary settings including care homes and undertake triage, provide expert assessment and use advanced clinical reasoning skills to determine differential diagnosis of frail older people referred to the service by primary care colleagues.
They will hold their own caseload and be responsible for the effective treatment and management of service users in order to maximise their independence and functional ability within their own home to prevent inappropriate admissions to hospital. They will have the opportunity to advocate for the value of physiotherapy within the team and provide training to other professional groups and HCSW in the team.
A key feature of the post will be to provide advice and support to family and carers and develop broad educational resources for use in care home settings. To take a lead role in the post-graduate education of physiotherapy colleagues and other members of the multidisciplinary team and work closely with the community physiotherapy team and wider At Home services e.g. Integrated Response and Reablement Teams.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
