An opportunity has arisen for a Health Intervention Worker Clinical Specialist to join the Integrated Family Support Team (IFST).
The IFST is a multi-agency team set up to work with children and families who are affected by parents/carers, drug and/or alcohol misuse.
The post has been developed in partnership between Rhondda, Cynon, Taf and Merthyr Tydfil County Borough Council’s and Cwm Taf Health Trust.
The role involves providing direct intervention to families who meet the criteria for intensive family support working across Rhondda, Cynon, Taf and Merthyr Tydfil. They will be responsible for assisting the family in creating a safe environment by reducing the level of risk and ensuring positive outcomes for the most vulnerable children.
The project will fast track clients into specialist treatments and develop innovative working relationships across multi professional services.
The post holder must have sufficient post registration experience as a Health Visitor or Community Mental Health Nurse, have excellent assessment and care planning skills and be able to manage their own caseload. Experience of working confidently with multi-agency services is essential.
The ability to speak Welsh is desirable for this post.
